The HCN Unit is an interagency unit that supports staff and managers across health, disability, education and Child, Youth and Family to identify, plan and better meet children’s needs when they are high and complex. The HCN Unit provides tools, resources and information to support interagency working and, where necessary, funding for the purchase of additional services.

- An updated version of the Interagency Plan form is now available online, along with instructions for using the interagency plan form.
