Team Planning Resources

The HCN Unit has developed team planning resources that will guide interagency teams as they work to develop plans for children and young people with complex needs.

The team planning resources include a series of booklets to help collect and collate the views of the young person, their whānau / caregiver, and practitioners from different agencies so that each team member has the opportunity for their views to be clearly represented. The team analysis worksheet helps the team identify what’s working well, and what needs to change.

The resources help teams to:

  • analyse and prioritise areas to be addressed
  • set specific, measurable goals
  • identify appropriate interventions
  • be clear about how progress will be measured and monitored.

What people are telling us
We’re already receiving positive feedback about the team planning resources.

“I have found using the team planning resources extremely valuable. I am using the guide for the first time and I can only compare it to my experience of having written two previous plans without such a tool”. – Services Coordinator

“Being of a somewhat pragmatic bent, I have been really surprised how well the guide enables the development of shared understandings, collection of information and at the same time a rewarding team… for all the members of the team this has been a very positive and hugely beneficial experience. [Young person’s name] participation in meetings was very moving and he participated to a degree that surprised all those who knew him. He shared insights, his frustrations and fears, and his hope for the future were expressed at times with a clarity of thought that instilled a huge sense of optimism for change.” – Services Coordinator
