Team Planning Resources
The HCN Unit has developed team planning resources that will guide interagency teams as they work to develop plans for children and young people with complex needs.
The team planning resources include a series of booklets to help collect and collate the views of the young person, their whānau / caregiver, and practitioners from different agencies so that each team member has the opportunity for their views to be clearly represented. The team analysis worksheet helps the team identify what’s working well, and what needs to change.
The resources help teams to:
- analyse and prioritise areas to be addressed
- set specific, measurable goals
- identify appropriate interventions
- be clear about how progress will be measured and monitored.
