Key roles and responsibilities

The services coordinator is contracted independently and has a key role in supporting the interagency team to work together to apply for, develop and implement a successful interagency plan.

The services coordinator works closely with the HCN Advisor, the Lead Agency Manager and the finance contact, and it is important they understand each other's roles and responsibilities.

To help services coordinators get started in their roles, here is some useful information.

Developing an application
3.

Services coordinators are responsible for collating information from each agency and writing the application for HCN funding.

Developing an interagency plan
4.

The services coordinator facilitates the interagency team to develop the interagency plan, using the team planning resources and supported by the HCN Advisor.

Developing the initial budget
5.

After the team has completed its analysis, identified priority goals and interventions, it is time to complete the interagency plan funding template. The funding request accompanies the interagency plan when it is sent to the Plan Approval Panel.

It is helpful to have each intervention in a separate line: the Benchmark Pricing Tool provides guidance about the price bands the HCN Unit can expect to pay for different kinds of input.

If a child is going to be living with a service provider for which there is not a standard agreement, the provider should submit their proposed budget using the provider budget template.

Private providers

If the interagency plan is to use private providers, it is a good idea to have them provide a quote and to attach their quote to the interagency plan and budget.

Monitoring the plan and financial process

The lead agency manager is accountable for managing the approved HCN funding, and they usually appoint a finance contact to do this on their behalf.

The services coordinator has a role in assisting the finance contact and lead agency manager by monitoring the payments against the plan.

Final review and other processes

The final review form is designed to assist the team in reflecting on the child / young person's progress while supported by an interagency plan, noting what has gone well and what could have been done differently.
